Helicobacter pylori is a universally distributed bacterium which affects more than half of the world population. The infection is associated with the development of various diseases of the upper gastrointestinal tract, besides extradigestive diseases. This book is a comprehensive overview of contributors on H. pylori infection in several areas. Its chapters were divided into sections concerning general aspects of H. pylori infection, immunopathology and genetic diversity, questions regarding possible routes of bacterium transmission, the importance of the strains characteristics in the development of gastric cancer and the possibilities of prevention, H. pylori infection in children, the possible association between its infection and extradigestive diseases, and the principal therapeutic regimens of bacterium eradication, considering the antimicrobial resistance.

A century ago the Italian pathologist Bizzozero described the relationships between spiral bacteria and the mammalian gastro intestinal tract. Since 1982, when Helicobacter pylori was discovered, gastroduodenal disease have been completely revised as a con sequence of the results of basic and clinical research in this field. Progress in understanding the pathogenesis of this bacterium has been made by studying H. pylori infection in animal models. More specific diagnostic tools have been developed using new molecular biology techniques. Future trends are directed towards preparing a specific H. pylori vaccine. A new classification for gastritis, the Sydney System, including H. pylori gastritis, was proposed in 1990. As concerns the clinical approach to peptic ulcer disease in the 1990's, the majority of authors agree on the importance of H. pylori eradication. Moreover, recent clinical studies suggest that H. pylori infection can be associated with other gastroduodenal diseases, such as non ulcer dyspepsia and gastric cancer. Multicenter trials to standardize serological methods and evaluate the efficacy of new antimicrobial therapy schedules are planned throughout different European countries. The fourth Workshop of the European Helicobacter Pylori Study Group was held in Bologna, Italy, in November 1991. Two years before Bologna University celebrated its ninth centennial, giving evidence of being the oldest University in the modern world. Thus the H. pylori story that has continued for more than a century has been discussed once again at the University with the oldest tradition in the world.

This book provides a detailed review of the most recent pathogenetic, diagnostic and therapeutic approaches in connection with Helicobacter pylori, along with priority areas in research and trends in clinical practice to address this significant pathogen acknowledged as the causative agent in a wide range of diseases, including gastric adenocarcinoma, MALT lymphoma and immune thrombocytopenic purpura (ITP). Medical authorities worldwide now recommend a “test and treat” strategy for this bacterial infection. Eradication therapy approaches range from general mass eradication therapy to tailor-made therapy for antibiotic-resistant refractory infection. In regions with high prevalence of H. pylori infection, the infection itself would be considered a major parameter to predict the risk of gastric adenocarcinoma development. The main interest has shifted recently to the risk stratification strategy for gastric cancer development and to the wide-ranging application of its serological prevention program. This book will benefit all gastroenterologists, hematologists, dermatologists, primary care doctors, epidemiologists, physicians who related with community hygiene, as well as basic scientists, particularly microbiologists, bacteriologists, pathologists, molecular biologists, biochemists, immunologists and oncologists.

Helicobacter pylori is a universally distributed bacterium that affects more than half of the world population. H. pylori infection causes persistent inflammation with different clinical outcomes in humans, including chronic gastritis, peptic ulcer disease, and gastric cancer. The infection has also been associated with several extradigestive disorders. In this book, there is a comprehensive overview of contributors on H. pylori infection in diverse areas, including virulence factors of H. pylori and their importance for the clinical outcome of the diseases, discussions about the principal therapeutic regimens of bacterium eradication, also considering the antimicrobial resistance. H. pylori is clearly a very interesting bacterium and studies and discussions about its aspects are welcome to the medical and scientific communities.

Infection by Helicobacter pylori has for many years been one of the medical conditions most studied not only in the gastroenterological context. This is because, in recent years, H. pylori has also been involved in a number of extra-intestinal diseases. In this book, we have tried to involve the biggest international experts on the subject. Some authors have tried to provide a complete view of the most common and widely used diagnostic methods, both invasive and non-invasive, also explaining the unusual techniques and the most innovative complementary methods not yet used in clinical practice. Other international experts have tried to describe the latest findings on pathogenesis and the various clinical manifestations of infection by H. pylori in the various systems. One chapter is reserved for H. pylori infection in childhood, an important period especially in transmission of the infection, explaining the various international guidelines for correct diagnosis and treatment. The authors have also provided the latest evidence regarding the various methods of eradication therapy, with a critical analysis of the reasons that compel some researchers to believe that eradication at all costs is not always correct and useful for humans. The book also deals with the current and latest knowledge regarding the progress and the difficulties in the development of a vaccine against H. pylori infection, the implementation of which could solve the problem of the difficulty in the eradication linked mainly to the onset of antibiotic-resistance.

This book is a comprehensive overview of invited contributions on Helicobacter pylori infection in gastritis and gastric carcinogenesis. The first part of the book covers topics related to the pathophysiology of gastric mucosal defense system and gastritis including the gastroprotective function of the mucus, the capsaicin-sensitive afferent nerves and the oxidative stress pathway involved in inflammation, apoptosis and autophagy in H. pylori related gastritis. The next chapters deal with molecular pathogenesis and treatment, which consider the role of neuroendocrine cells in gastric disease, DNA methylation in H. pylori infection, the role of antioxidants and phytotherapy in gastric disease. The final part presents the effects of cancer risk factors associated with H. pylori infection. These chapters discuss the serum pepsinogen test, K-ras mutations, cell kinetics, and H. pylori lipopolysaccharide, as well as the roles of several bacterial genes (cagA, cagT, vacA and dupA) as virulence factors in gastric cancer, and the gastrokine-1 protein in cancer progression.
