Helicobacter pylori is an universally distributed bacterium which affects more than half of the world population. H. pylori infection causes persistent inflammation with different clinical outcomes in humans, including chronic gastritis, peptic ulcer disease and gastric cancer. The infection has also been associated with several extradigestive disorders. In this book there is a comprehensive overview of contributors on H. pylori infection in diverse areas, including virulence factors of H. pylori and their importance for the clinical outcome of the diseases, discussions about the principal therapeutic regimens of bacterium eradication, also considering the antimicrobial resistance. H. pylori is clearly a very interesting bacteria and great studies and discussions about all its aspects are welcome to the medical and scientific communities.

A century ago the Italian pathologist Bizzozero described the relationships between spiral bacteria and the mammalian gastro intestinal tract. Since 1982, when Helicobacter pylori was discovered, gastroduodenal disease have been completely revised as a con sequence of the results of basic and clinical research in this field. Progress in understanding the pathogenesis of this bacterium has been made by studying H. pylori infection in animal models. More specific diagnostic tools have been developed using new molecular biology techniques. Future trends are directed towards preparing a specific H. pylori vaccine. A new classification for gastritis, the Sydney System, including H. pylori gastritis, was proposed in 1990. As concerns the clinical approach to peptic ulcer disease in the 1990's, the majority of authors agree on the importance of H. pylori eradication. Moreover, recent clinical studies suggest that H. pylori infection can be associated with other gastroduodenal diseases, such as non ulcer dyspepsia and gastric cancer. Multicenter trials to standardize serological methods and evaluate the efficacy of new antimicrobial therapy schedules are planned throughout different European countries. The fourth Workshop of the European Helicobacter Pylori Study Group was held in Bologna, Italy, in November 1991. Two years before Bologna University celebrated its ninth centennial, giving evidence of being the oldest University in the modern world. Thus the H. pylori story that has continued for more than a century has been discussed once again at the University with the oldest tradition in the world.

Infections caused by Helicobacter pylori (H. pylori) bacteria constitute the primary factor behind the development of stomach ulcers. However, a significant number of individuals harboring H. pylori may remain unaware of their infection due to its typically asymptomatic nature. In cases where ulcer-related symptoms manifest, prompt treatment to eradicate the infection and eliminate the bacteria is advisable. Failure to address ulcer symptoms in a timely manner may lead to complications, and persistent H. pylori infections can elevate the risk of specific types of stomach cancer. Fortunately, there is optimistic news as H. pylori is treatable, allowing for the healing of ulcers resulting from the infection and a reduction in the risk of complications and future ailments. Primary treatments for H. pylori involve the use of medications such as antibiotics and acid reducers. While multiple treatment sessions may be necessary, it is feasible to cure H. pylori infections with the appropriate intervention.

Now that Helicobacter pylori is generally accepted as a key aetiological agent in gastric cancer as well as the main agent in peptic ulcer, it can claim to be the most important new discovery in clinical gastroenterology of the last decade, and yet there is no up-to-date book available on the subject that is designed primarily for the clinical gastroenterologist. This book aims to fill that niche. It should also be of interest to the basic scientist, to those providing a clinical laboratory service (microbiologists and histopathologists), and to epidemiologists and others involved in clinical research.

Helicobacter pylori is a universally distributed bacterium which affects more than half of the world population. The infection is associated with the development of various diseases of the upper gastrointestinal tract, besides extradigestive diseases. This book is a comprehensive overview of contributors on H. pylori infection in several areas. Its chapters were divided into sections concerning general aspects of H. pylori infection, immunopathology and genetic diversity, questions regarding possible routes of bacterium transmission, the importance of the strains characteristics in the development of gastric cancer and the possibilities of prevention, H. pylori infection in children, the possible association between its infection and extradigestive diseases, and the principal therapeutic regimens of bacterium eradication, considering the antimicrobial resistance.

This book gathers a wealth of contributions on the virulence factors and pathogenic mechanism of Helicobacter pylori, prepared by leading international experts. In addition, it explores the epidemiology, diagnosis, treatment with drugs and probiotics, and prophylaxis by vaccination, reflecting the latest advances. H. pylori is a Gram negative microaerophilic bacterium that can produce various gastric diseases including gastritis, gastroduodenal ulceration, gastric cancer and gastric MALT lymphoma. Although efforts to combat H. pylori using a combination of proton pump inhibitor and several antimicrobial drugs have significantly decreased the burden of these gastric diseases, the microbial epidemiology and gastric pathogenesis following H. pylori infection are still not fully understood. Given its scope, the book offers a valuable resource not only for basic microbiologists, but also for researchers in the fields of pathology, biochemistry and genomics, as well as medical students/scientists.

Part I (5 chapters) reviews the epidemiology of H. pylori infection, H. pylori infection in Asia, gastric adenocarcinoma, antimicrobial resistance and approaches to treatment, and extragastric manifestations of H. pylori infection. Part II (4 chapters) describes H. pylori-induced acquired immunity and immunoregulation, host genetic factors and susceptibility to H. pylori pathogenesis, innate immune initiators and effectors in H. pylori infection, and H. pylori vaccines. Part III (4 chapters) discusses the importance of H. pylori lipopolysaccharides in gastric adaptation and pathogenesis, H. pylori virulence factors, H. pylori adhesion to the gastric surface, and helicobacteromics. This book is an essential resource for researchers, students and medics in infectious and Helicobacter-associated diseases.
