This text aids medical students and trainee doctors in developing their knowledge of the conditions covered and improving their ability to clinically assess patients and formulate a management plan. Once doctors are comfortable in interpreting these graphs, they can use them in consultations with patients about their kidney condition and so support self-management by patients, an increasingly important aspect of healthcare. Kidney diseases and the speciality of nephrology have traditionally been regarded as difficult to understand. This is compounded by the use of complex definitions and terminology. These complex definitions result from a need to categorise kidney diseases according to the way kidney function changes over time, and then to express this in numbers and words. Central to the assessment of patients is the measurement of kidney function. In our teaching of medical students and trainee doctors, as well as in our clinical practice and interaction with GPs and consultant colleagues, we have found that viewing graphs of patients’ kidney function (estimated glomerular filtration rate, or eGFR) against time is a much easier way of understanding kidney diseases. It provides the patient’s ‘kidney history’ and leads into a description of the natural history and management of their condition. We were surprised to find that this graphical approach is not used routinely in teaching or in daily clinical practice. We therefore believe there is a need for a book that uses this approach and we thus sought to compile a collection of illustrative case studies covering an array of disease categories, together with patients’ eGFR readings.

Kidney disease touches nearly 14% of the population. Of the 661,000 Americans at the end stage of chronic kidney disease, 468,000 require dialysis and 193,000 have an effective kidney transplant. With the increase in lifestyle-related diseases like diabetes and hypertension the prevalence of Chronic Kidney Disease (CKD) is increasing exponentially and with it is increasing the burden of its ominous consequence, End Stage Renal Disease (ESRD). While ESRD is no longer a death sentence with the advent of efficient renal replacement therapies and the success of renal transplantation, factors such as high cost of these procedures, limited availability of donated kidneys and not enough number of centers equipped with these facilities puts the effective management of ESRD beyond the reach of an average person many-a-times. Additionally the presence of co-existing diseases that contribute to and sometimes complicate the renal impairment as well as delayed referral of the patients to nephrologists also makes matters worse. ESRD is not just a medical but also a social and economic condition that devastates the person and his/her entire family. Hence, early detection and effective prevention of progression of CKD in early stages to advanced CKD and ESRD is the call of the day. In fact, understanding the pathophysiology of the condition and adopting methods of primordial prevention in populations at risk may be desirable to ensure reduction in the incidence of CKD. In those with established CKD, a proactive approach to manage the disease manifestations and limit the ravages of other comorbidities is desirable. For those in advanced stages of CKD, the institution of an appropriate renal replacement therapy individually suited to the patient keeping in view the medical status, lifestyle requirements, economic viability and social acceptability should be advised. This research brief examines chronic kidney disease of unknown etiology (CKDu) in Sri Lanka. A new and growing public health challenge that has emerged in Sri Lanka, CKDu is epidemic in selected geographic areas of the country, primarily in its central and eastern dry zones. CKDu is not attributed to diabetes, hypertension, or other factors commonly associated with chronic kidney disease. A number of risk factors point to environmental triggers for the onset of CKDu, but evidence so far is insufficient to accurately pinpoint the potential cause or causes. Small studies conducted to date attempt to associate CKDu with agrochemicals, ayurvedic (traditional) medicines, water quality, or other environmental or lifestyle factors. This complex emerging health crisis requires an interdisciplinary approach, following strict field protocols and a rigorous environmental health risk assessment framework. Research and analysis to better understand CKDu are important for Sri Lanka and have global implications for understanding of similar geographic CKDu “hot spots” in Central America, Eastern Europe, and South Asia.
