WHO approved more hepatitis direct-acting antivirals (DAAs) for the treatment of infection with the hepatitis C virus (HCV) by 2020 which helped low- and middle-income countries increase the number of registered treatments, reduce prices of a full treatment under US$100 – update is as low as $60 for three months, and strive towards hepatitis C elimination. For the first time, this year’s report includes information on hepatitis C diagnostics. With a focus on selected countries with diverse HCV epidemics, the report provides updates on the various dimensions of access to HCV diagnostics and pharmaceutical products, including product pricing, the regulatory environment and patent status, which together shape the national hepatitis response in different settings. It highlights key areas for action by ministries of health and other government decision-makers, pharmaceutical manufacturers and technical partners. Globally, at the end of 2017, only 5 million – or 7% – of the 71 (62–79) million people chronically infected with HCV had cumulatively received treatment with DAAs. As countries continue to tackle the disease burden and service disruptions caused by the COVID-19 pandemic, it is critical to ensure that the recent momentum and gains in the response to hepatitis C are not lost. Against this backdrop, this report is a strong reminder to continue to invest in building resilient and sustainable health systems that are focused on universal health coverage.

The field of HCV therapeutics continues to evolve rapidly and since the World Health Organization (WHO) issued its first Guidelines for the screening care and treatment of persons with hepatitis C infection in 2014 several new medicines have been approved by at least one stringent regulatory authority. These medicines called direct-acting antivirals (DAAs) are transforming the treatment of HCV enabling regimens that can be administered orally are of shorter duration (as short as eight weeks) result in cure rates higher than 90% and are associated with fewer serious adverse events than the previous interfere on containing regimens. WHO is updating its hepatitis C treatment guidelines to provide recommendations for the use of these new medicines. The objectives of these WHO guidelines are to provide updated evidence-based recommendations for the treatment of persons with hepatitis C infection using where possible all DAA-only combinations. The guidelines also provide recommendations on the preferred regimens based on a patient?s HCV genotype and clinical history and assess the appropriateness of continued use of certain medicines. This document also includes existing recommendations on screening for HCV infection and care of persons infected with HCV that were first issued in 2014. The key audience for these guidelines are policy-makers in low- and middle-income countries who formulate country-specific treatment guidelines and who plan infectious disease treatment programmes and services in addition to those people responsible for delivering treatment. The guidelines are appropriate for all countries including high-income countries.

In several liver diseases, the underlying cause cannot always be eliminated, i.e. the progression of liver disease cannot be prevented. This is particularly true for non-responders to the treatment of chronic hepatitis C (HCV). It is relevant for more than 40% of patients with HCV genotype 1 and up to 20% of patients with genotype 2 or 3. Several approaches are now underway to prevent or ameliorate mechanisms of disease progression. In Asia, and particularly in Japan, Glycyrrhizine-SNMC has been widely used for this purpose. At present, SNMC is under clinical evaluation in Europe. This book, the proceedings of the Falk Workshop on 'Prevention of Progression in Chronic Liver Disease - An Update on SNMC (Stronger Neo-Minophagen C)' (Part IV of the XII Falk Liver Week, in honour of Hans Popper's 100th birthday) held in Freiburg, Germany, on 22 October 2003, is devoted to the understanding of basic mechanisms of fibrogenesis, the state of the art of non-invasive markers of disease progression that may substitute liver biopsy in the future, and of other predictive parameters indicating disease progression, in particular the development of hepatocellular carcinoma. All these problems are particularly relevant for chronic hepatitis C. A better characterization of so-called non-responders for the treatments available is crucial for the development of new strategies preventing or postponing disease progression. Such strategies should influence fibrogenesis and, thus, cirrhosis formation as well as the development of life threatening complications of liver cirrhosis, including hepatocellular carcinoma. In this volume, the available information on SNMC as an approach to prevent progression of chronic liver disease is discussed by several internationally renowned authors who report on the mechanisms of liver fibrogenesis and fibrolysis, on non-invasive markers of disease progression, and on hepatocellular carcinoma as one of the major complications of liver cirrhosis nowadays. Finally, experts from Japan report on their knowledge and experience concerning the drug SNMC/Glycyrrhizine in the prevention of cirrhosis and HCC, and first study results from Europe are presented.

This policy brief focuses on the new recommendations on treatment of adolescents and children ages ≥3 years with chronic HCV infection.
