The aim of this operational handbook is to provide practical guidance on the implementation of the World Health Organization (WHO) policy recommendations on the prevention and management of TB in children and adolescents under programmatic circumstances and at different levels of the health system. The practical guidance aims to inform the development or revision of national policies and related implementation guidance (e.g. handbooks, standard operating procedures) on the management of TB in children and adolescents. This handbook can also help countries adequately plan for the uptake of interventions to better address the specific needs of children and adolescents with or at risk of TB. It can contribute to national efforts to build capacity among national and subnational programme managers and among health workers at all levels of the health care system. The target audience for this handbook includes NTPs and other child health programmes that provide care for children with or at risk of TB, including maternal, newborn, child and adolescent health programmes, HIV services, and PHC programmes. The handbook also targets paediatricians and other health care workers (HCWs) in the public and private sectors, school health services, civil society and community-based organizations, and health care educators.

Addressing comorbidities and risk factors for tuberculosis (TB) is a crucial component of the World Health Organization (WHO)’s End TB Strategy. This WHO operational handbook on tuberculosis. Module 6: tuberculosis and comorbidities aims to support countries in scaling up people-centred care, based on the latest WHO recommendations on TB and key comorbidities, and drawing upon additional evidence, best practices and inputs from various experts and stakeholders obtained during WHO processes. It is intended for use by people working in ministries of health, particularly TB programmes and the relevant departments or programmes responsible for comorbidities and health-related risk factors for TB such as HIV, diabetes, undernutrition, substance use, and tobacco use, as well as programmes addressing mental health and lung health. This handbook provides practical advice on how to implement the WHO recommendations on TPT at the scale needed to achieve national and global impact, and complements the second edition of the WHO guidelines on TPT being released concomitantly. It covers critical steps in the programmatic management of TPT following the cascade of preventive care: identifying individuals at highest risk, screening and ruling out TB disease, testing for infection, choosing the best suited treatment regimen, managing adverse events, supporting adherence and monitoring programmatic performance. The dosage schedules for TPT regimens have been completely redone since the first edition of the handbook and examples from country and partner interventions have been included. The annexes include useful resources for advocacy and implementation, such as returns on investment from screening and preventive treatment, sample text for messaging, considerations for coordination and budgeting for national strategic planning and resource mobilization, and monitoring tools. Although its focus is on high TB and HIV burden settings, the advice in the handbook is also relevant for other contexts. The objectives of the 2022 consolidated guidelines are: to provide policy makers and implementing partners with evidence-based recommendations on the cascade of care for children and adolescents; to support the implementation of activities to prevent TB among children and adolescents at risk; to improve TB case detection and treatment outcomes in children and adolescents with TB using effective models of care; and to contribute to reductions in TB related morbidity and mortality in children and adolescents in line with global targets including those in the UN Sustainable Development Goals, the WHO End TB Strategy and the Political Declaration of the United Nations General Assembly High-Level Meeting on the Fight against Tuberculosis. To ensure that countries can effectively benefit from digital health investments, “digital adaptation kits” (DAKs) are designed to facilitate the accurate reflection of WHO’s clinical, public health and data use guidelines in the digital systems that countries are adopting. DAKs are operational, software-neutral, standardized documentations that distil clinical, public health and data use guidance into a format that can be transparently incorporated into digital systems. For this particular DAK, the operational requirements are based on systems that provide the functionalities of digital tracking and decision support (DTDS) and include components such as personas, workflows, core data elements, decision-support algorithms, scheduling logic and reporting indicators. Web annexes provide certain components in additional detail including: data dictionary (Web Annex A), decision-support logic (Web Annex B), indicator definitions (Web Annex C), and functional and non-functional requirements (Web Annex D). Data elements within the DAK (Web Annex A) are mapped to standards-based terminology, such as the International Classification of Diseases (ICD), to facilitate interoperability. This DAK focuses on providing the content requirements for a DTDS system for HIV care used by health workers in primary health care settings. It also includes cross- cutting elements focused on the client, such as self-care interventions.
