This book offers a detailed guide to MR-Linac, a unique and fast growing radiation treatment modality. MR-linac is new technology that is a fusion of an MRI and a linear accelerator on the same gantry. It can change both target volume delineation and tumor visualization in real time using MR-cine images and treatment. Tumor location changes moment to moment as radiation is delivered, but this cannot be visualized in current radiation therapy practices. This new and rapidly growing technology can provide adaptive therapy that was not possible before. This book presents current knowledge on MR-linac technology, clinical practices, and ultimately patient outcome where dose escalation is not possible due to limiting normal tissue structures in the vicinity of tumor. There are two commercial MR-linac machines under consideration and both will be covered in detail. The book is divided into four sections. The first gives a general introduction to MR-Linac, covering the role of MRI inradiation oncology, the clinical necessity of this technology, and patient selection. The next section details the physics and technology of MR-Linac, covering image sequence, motion management, and treatment planning. Section three offers the clinical applications of MR-Linac and is divided by body area, including lung, prostate, and breast. Finally, the fourth section looks to the future and what this technology can mean for radiation oncology. This is an ideal guide for radiation oncologists, medical physicists, and relevant trainees.

MRI-LINAC based radiation treatments are currently being used for selected patient populations and body regions, and knowledge related to the outcomes of this modality is progressing. The outcome and long-term side effects in comparison with other modern LINAC are now being studied. As contouring, planning, quality assessment, and treatment assessment require new techniques for the use of all team members, including technicians, dosimetrists, and physicians, research reporting on these factors and each stage of the process is needed for medical professionals. Furthermore, as physicians report a low incidence of acute side effects, with almost none for long-term follow-up, this knowledge should be published in the radiation milieu. The goal of this Research Topic is to collect and summarise the growing knowledge from institutions using MRI-LINAC, in order to share the obstacles, solutions, learning curves, and innovations of this new treatment modality.

This new edition is a fully updated, comprehensive review of stereotactic radiosurgery (SRS) and stereotactic body radiation therapy (SBRT): its physics, clinical evidence, indications, and future directions. The utilization of stereotactic radiosurgery (SRS) and stereotactic body radiation therapy (SBRT) is increasing internationally because of several factors. First, it offers patients a local treatment option that has demonstrated effectiveness similar to traditional surgery without the morbidity of general anesthesia and open surgical resection. Second, recent advancements in the quality of scientific evidence supporting a SRS or SBRT-containing approach in patients continues to evolve and demonstrate favorable disease-specific outcomes with little, if any, toxicity in various anatomic disease sites and for various conditions including cancer, benign tumors, and other psychiatric and neurologic conditions. Third, and most provocatively, is the notion that definitive local therapy (i.e. SRS or SBRT) in patients with cancer can boost the immune system to fight cancer in other sites throughout the body. While traditional medical knowledge would suggest that all patients with metastatic cancer are incurable, there is a mounting body of evidence that there is a subset of these patients that can be cured with definitive SRS or SBRT. This volume thus delves into each of these benefits and aspects of treatment, guiding physicians to the best treatment plan for their patients. Expert, international authors provide guidelines for SRS and SBRT use by clinicians. Chapters are divided into six main sections: Radiobiology of Radiosurgery and Stereotactic Body Radiation Therapy, Intracranial Radiosurgery Technique, Intracranial Radiosurgery by Indication, Stereotactic Body Radiation Therapy Technique, Stereotactic Body Radiation Therapy by Indication, The Future of Radiosurgery and SBRT. Overall physics are explained, as well as specific considerations for particular surgical tools (including the Leksell Gamma Knife and Accuray CyberKnife), techniques (including fractionated and charged particle radiosurgery), and anatomic sites (including brain metastases, pituitary tumors, and the prostate). Since the first edition published, the field has grown significantly. There is now significant new data to support preoperative radiosurgery, increased indications in metastatic cancers, as well as integration with new drug therapies and imaging techniques. Each chapter is thus fully updated with the latest in medical advancements and new scientific research. Detailed images and charts enhance the chapters. This book provides physicians with a single, practical resource incorporating both of these broad categories of treatment, SRS and SBRT, and better defines the current role and the direction of radiosurgery.

Prostate Mri Essentials written by Temel Tirkes and has been published by Springer Nature this book supported file pdf, txt, epub, kindle and other format this book has been release on 2020-06-09 with Medical categories.

This book is a basic, practical guide to performing and interpreting state-of-the-art prostate MRI, utilizing the latest guidelines in the field. Prostate MRI has become one of the fastest growing examinations in the radiology practice, and this demand has continuously increased within the past decade. Since it is relatively new, MRI of the prostate is predominantly being performed at academic institutions, however there is a growing demand within the lower-tier health care institutions to offer this examination to their patients. This is an ideal guide for radiologists who want to enhance or initiate prostate MRI service for their referring clinicians and as a manual for technologists and those who are in training. Prostate cancer is the second leading cause of cancer death in men, exceeded only by lung cancer. The best predictor of disease outcome lies with correct diagnosis, which requires precise imaging and diagnostic procedures aided by prostate MRI. Urologists, medical oncologists and radiation oncologists all agree that multi-parametric prostate MRI is essential for evaluation of prostate cancer. However, the technical aspects of prostate MR imaging are not as straightforward as for the other imaging modalities and constantly evolving. Its small size presents a real challenge to the radiologist, who needs to do the T2 and diffusion weighted images and perform a dynamic contrast enhanced sequence correctly. These images may also need to be analyzed on an independent workstation. Due to the absence of a current reference manual, when a radiologist wants to establish a prostate imaging service, he/she needs to attend dedicated prostate MR workshops or dive into the literature search alone, only to get more confused about what to do and how to do it. With this book, expert authors were asked to give clear guidance to those who want to enhance or initiate their prostate imaging service. With this much-needed, concise, practical guidance, radiologists can perform and interpret multi-parametric prostate MRI in a standardized fashion, in concordance with PI-RADS v2.1 that can be applicable to all available hardware platforms (GE, Philips, Siemens, Toshiba). Additionally, they can perform post-processing for possible targeted biopsy and interpret post-therapy and PET studies. The book discusses imaging protocols (planning and prescription) and sequence parameters with representative images for each MRI sequence. This handbook-style practical manual can be used in the radiology reading room by those interpreting the MR exam as a reference as well as at the MRI scanner by the technologists as a guide. Coverage of basic prostate anatomy, pathology, Urologists’ point of view, MRI guided radiation treatment planning and molecular imaging is also included. Throughout the book, authors will discuss basics, pitfalls, and provide tips in image acquisition and interpretation, alongside several case examples.

This new edition summarizes the data and techniques for hypofractionation and stereotactic radiation in a clinically-accessible way. Hypofractionated radiation therapy, which consists of larger-dose radiation treatments that are given over a shorter time period compared to conventional radiation fraction sizes, is used to treat a variety of cancers, including prostate, breast, lung, and colorectal. Conventional radiation therapy and hypofractionated radiation therapy often have different effectiveness rates for cancer treatment and have different impacts on normal tissues in terms of causing toxicity. There is a significant and continually evolving body of literature on the use of different dosing regimens to treat a variety of cancers, and radiation oncologists need to keep up with the various dosing schedules, the effect of each regimen on cancer control in different cancers, and how the different schedules affect each organ in terms of toxicity. This book provides concise, up-to-date information ranging from commonly-used dose-fractionation schemes for hypofractionated and stereotactic body radiotherapy to simulation and treatment specifications to published safety and efficacy data. Chapters additionally examine the biological rationales for the efficacy of hypofractionated radiation; present clinical studies that demonstrate the efficacy and safety of hypofractionated radiation treatment in a variety of cancers; and describe the advances in technology that have allowed hypofractionated radiation to be safely given. All chapters are fully revised with the latest treatments and updated effectiveness and toxicity of previously discussed treatments, all based on recent clinical trials and large analytical studies. This is an ideal guide for radiation oncology clinicians and trainees.
