Why has the response to HIV/AIDS been unique? How did civil society organizations gain access to global decision-making forums to demand exceptional attention and resources for HIV/AIDS? This book seeks to answer these questions, among others, through a critical international relations approach that enquires into the role of civil society in global health governance. It documents how civil society forged the initial response to HIV/AIDS within a rights-based paradigm, and built international networks. It analyses why civil society was able to gain the right to participate in global health institutions and assesses what influence civil society representatives have within these institutions, particularly focusing on outcomes related to institutional legitimacy and downward accountability. It then discusses changes in the broader political economy of global health and how HIV/AIDS organizations have, or have not, adapted to these shifts. Finally the book tells the story of the many struggles civil society organizations have engaged in to advance a rights-based response to HIV/AIDS, the transformations achieved and the resistance experienced.

How resources are being used to fund the community response to human immunodeficiency virus (HIV) and acquired immune deficiency syndrome (AIDS) is of considerable interest to the donor community and governments. In the past decade, international funding for the HIV and AIDS response provided by governments rose from about US$1 billion to US$8.7 billion; donors increasingly shifted their financial support toward funding community responses to this epidemic. Yet little is known about the global magnitude of these resource flows and how funding is allocated among HIV and AIDS activities and services. Although some studies have been carried out to gather information on the community response by civil society organizations (CSOs), most of them provide only partial information limited to a specific intervention (for example, orphan support) or specific local communities. To address this knowledge gap, the report attempts to answer the following questions: How large is donor funding for community-based interventions that are run by either large nongovernmental organizations (NGOs) or smaller community-based organizations (CBOs)? How do the funds reach various types of CSOs? What are CSOs' other sources of funding, and to what extent are the CSOs dependent on donor funding? How are these funds used for by CSOs? Are there differences among different types of CSOs working on HIV and AIDS?

This book studies the role of civil society organisations in the fight against HIV/AIDS in Russia. It looks at how Russia’s HIV/AIDS epidemic has developed into a serious social, economic and political problem, and how according to the United Nations Programme on HIV/AIDS (UNAIDS), Russia is currently facing the biggest HIV/AIDS epidemic in all of Europe with an estimated number of 980,000 people living with HIV in 2009. The book investigates civil society organisations’ contribution to social change and civil society development in post-Soviet Russia, and thus situates a specific type of civil society actors into a broader socio-political context and questions their ability to represent civic interests, particularly in the field of social policy-making and health. AIDS activists are often romanticized as extremely noble and selfless. However, the relationships among HIV support group members highlighted in Landscapes of Activism are hardly utopian or ideal. At first, the group has everything it needs, a thriving membership, and support from major donors. Soon, the group undergoes an identity crisis over money and power, eventually fading from the scene. As government and development institutions embraced activist demands—decentralizing AIDS care through policies of health systems strengthening—civil society was increasingly rendered obsolete. Charting this transition—from subjects, to citizens, and back again—reveals the inefficacy of protest, and the importance of community resilience. The product of in-depth ethnography and focused anthropological inquiry, this is the first book on AIDS activists in Mozambique. AIDS activism’s strange decline in southern Africa, rather than a reflection of citizen apathy, is the direct result of targeted state and donor intervention.
