This volume explores the myriad of techniques and methodological approaches that are being used in breast cancer research. The authors critically evaluate of the advantages and disadvantages of current methodologies, starting with the tools available for understanding the architecture of the human breast, including its tissue and cellular composition. This book is a logical companion volume to Women at High Risk to Breast Cancer (Kluwer, 1989) edited by me previously. It distinguishes two aspects of current ap proaches to clinical breast cancer prevention. The first is the need to advise individ ual women on how they might reduce their personal risk, while the second is the design of measures aimed at reducing the total incidence of breast cancer in the community. While the former is a problem faced daily by clinicians, the latter is a goal which will involve large scale, carefully planned interventional studies. Because knowledge of the risk factors for breast cancer is incomplete and clinical trial reports are scarce, there is as yet, no scientifically-based model for personal breast cancer prevention. Nevertheless, widespread publicity associated with breast screening programmes has created a large group of highly anxious women who have been informed that they are at higher than average risk to the disease. They are con cerned by the personal threat posed by a family history of the disease and by the al leged dangers of obesity, diet, alcohol, or the use of hormonal agents such as oral contraceptives or hormone replacement therapy.

The mysterious disease of cancer, including breast cancer, has plagued mankind since the dawn of recorded history. Regarding the elusive cause of the disease, the "Father of Medicine," Hippocrates of Athens (460–377 BC), wrote that, "For instability is characteristic of the humours and so they may be easily altered by nature and by chance." The enigma has persisted until today. In 1971, then President Richard Nixon signed the National Cancer Act and declared a "War on Cancer." He believed the counsel of scientists and physicians that if sufficient resources were committed to the fight, cancer could be virtually eliminated within 5 years. The prophesy failed. Although mortality from a few cancers, most notably leukemias, has been significantly reduced, carcinomas, cancers of the epithelium, which account for 80% of cancer deaths, remain unchanged. While tremendous advances have taken place in our understanding of the molecular and cellular mechanisms operant in cancer, it has proven exceedingly difficult to prevent the occurrence or to halt the progress of the disease. The very best therapy remains early detection while the primary tumor is small and localized to a single site, followed by removal of the offending growth by surgery and/or radiation. The great challenge of finding a cure confronts us yet, and it is effective intervention at the molecular level that offers our best hope. We still must find the "magic bullet."

The outlook for women with breast cancer has improved in recent years. Due to the combination of improved treatments and the benefits of mammography screening, breast cancer mortality has decreased steadily since 1989. Yet breast cancer remains a major problem, second only to lung cancer as a leading cause of death from cancer for women. To date, no means to prevent breast cancer has been discovered and experience has shown that treatments are most effective when a cancer is detected early, before it has spread to other tissues. These two facts suggest that the most effective way to continue reducing the death toll from breast cancer is improved early detection and diagnosis. Building on the 2001 report Mammography and Beyond, this new book not only examines ways to improve implementation and use of new and current breast cancer detection technologies but also evaluates the need to develop tools that identify women who would benefit most from early detection screening. Saving Women's Lives: Strategies for Improving Breast Cancer Detection and Diagnosis encourages more research that integrates the development, validation, and analysis of the types of technologies in clinical practice that promote improved risk identification techniques. The committee assembled for this event was asked to share insights and consider ways in which the objectives of the report could be achieved from the standpoint of what women need to know, the best models of screening programs, manpower, risk stratification, basic research, and payment. This symposium seeks to provide continuing food for thought and ideas for actions in support of breast cancer detection and diagnosis and saving women's lives.
