Our second Research Topic in this series, Computational tools in inferring cancer tissue-of-origin and molecular classification towards personalized cancer therapy, Volume II (https://fro.ntiers.in/14361) has over 8 accepted articles and further manuscripts currently under review. Due to the continued success of these Research Topics and the interest in the subject, we will launch a third volume on the same topic. Inferring cancer tissue-of-origin and molecular classification are two critical problems in personalized cancer therapy. It is known that there are about 5% cancers of unknown primary (CUP) site. These kinds of patients are under empirical chemotherapy, which leads to a very low survival rate. Thus, it is important to infer cancer tissue-of-origin. However, experimental methods usually fail to identify the exact tissue-of-origin even after the death of a patient, which provides a need for computational methods especially in the era of big biomedical data. Based on the finding that gene expressions of metastasis cancer cells are more similar to those of original tissue than metastasis tissue, there have been a few computational methods developed in this area. However, the accuracy of the methods is yet to be improved to assure a clinical usage. In addition to CUP, inferring cancer tissue-of-origin is also important in avoiding misdiagnosis even if the cancer origin is known.

The number of cancer cases is expected to rise due to longer life expectancy and global population growth. Cancer occurs when cells grow uncontrollably due to genetic mutations, enabling them to spread and evade the immune system. These cells interact with their environment, making the process complex. However, understanding genetic mutations can provide insights into cancer progression. Targeted therapies based on these mutations are sometimes effective, though their impact is often temporary. Genetic targeting has improved treatments for cancers like lung cancer and melanoma. The aim is to identify specific genetic traits in a patient’s cancer and use drugs that target them. A major challenge is overcoming resistance, as genetic differences within tumor cells complicate treatment. Studies suggest cancer cells evolve over time, similar to natural selection, creating a diverse cell population that resists single-treatment approaches. To address this, researchers are exploring drug combinations that target multiple aspects of cancer. Composite mutations, where multiple mutations occur within the same gene, are also a focus, though they are not yet fully understood. These mutations may play a key role in cancer's evolution and treatment resistance. With the rise of big data, machine learning offers the potential to personalize cancer treatment. By combining genetic, environmental, and lifestyle data, a detailed patient “landscape” can be created. This approach could help doctors make more informed treatment decisions. The goal of this thesis was to study cancer genes, their response to treatments, and identify optimal combination therapies that provide the best outcomes and quality of life. In Chapter 2, we analyzed cancer types and found that 5% of patients had composite mutations, which correlated with other mutations in the same chromosomal region.
