Im zweiten Teil wird die Struktur des CM-Prozesses anhand des Handlungskonzeptes und der verschiedenen Perspektiven und Ebenen von CM sowie die Durchführung in der Praxis vorgestellt. Dabei wird der "Expertenstandard Entlassungsmanagement in der Pflege" sowie Beispiele aus der Praxis beschrieben. Im Anschluss wird näher beleuchtet, welche Bedeutung die Einführung des CM-Ansatzes im Hinblick auf ihre Effektivität und Effizienz im EM hat.

How we reason with mathematical ideas continues to be a fascinating and challenging topic of research--particularly with the rapid and diverse developments in the field of cognitive science that have taken place in recent years.
