The Second International Symposium on Narcolepsy was held at Fairchild Auditorium, Stanford University, on 6-7 July 1985 under the presidency of Drs. William C. Dement and Christian Guillemi nault. It succeeded the First International Symposium on Narco lepsy held in La Grande Motte, France, organized by Pierre Pas souant in July 1975 in commemoration of the 100th anniversary of the publication of Jean B. E. Gelineau's paper which proposed the naming of narcolepsy. At the second narcolepsy symposium, many important research reports on both basic and clinical aspects of narcolepsy were given by investigators from many countries of the world. Audience inter est was particularly attracted by the section on the relationship be tween HLA and narcolepsy, in which recent evidence that almost all narcoleptic patients are HLA-DR2 positive was reported by in vestigators from Japan, England, France, Canada, and the United States. The close relationship between the HLA antigens, hitherto considered as immune-related genetic markers encoded by genes on human chromosome 6, and narcolepsy appeared to open a new approach not only for the research of narcolepsy but also for the mechanism of sleep in general. Publication of all these new findings on the association of HLA and narcolepsy was considered; an outline was worked out and all the groups agreed to prepare a contribution covering the various aspects of this topic.

Narcolepsy occurs during childhood in combination with cataplexy in one-third of the subjects. Symptoms may develop rapidly over a few weeks or months, with excessive daytime sleepiness and cataplexy being the most dramatic and observable symptoms. It can be secondary to brain tumors or several rare diseases, but in most cases narcolepsy with or without cataplexy is a primary condition, better explained by the selective loss of hypocretin neurons in posterolateral hypothalamus. A specific HLA allele, HLA-DQB1*0602, is involved and the disease is thought to be autoimmune. Tribbles 2-specific antibodies have been identified. Recent medications have greatly improved the symptoms and wellbeing of young patients. However, these treatments are delivered off-label in the pediatric population. There is an absolute necessity for well-conducted clinical trials in order to improve treatment in children and adolescents with narcolepsy and to evaluate clinical efficacy and good tolerance of medications. Nonpharmacological approaches are certainly helpful and should be promoted systematically, especially in very young children. Narcolepsy of recent onset in children or adolescents should be considered a therapeutic emergency, even though immunotherapy is still controversial.

utoimmunity is the downstream outcome of a rather extensive and coordinated series of events that include loss of self-tolerance, peripheral lymphocyte Aactivation, disruption of the blood-systems barriers, cellular infiltration into the target organs and local inflammation. Cytokines, adhesion molecules, growth factors, antibodies, and other molecules induce and regulate critical cell functions that perpetuate inflammation, leading to tissue injury and clinical phenotype. The nature and intensity of this response as well as the physiological ability to restore homeostasis are to a large extent conditioned by the unique amino acid sequences that define allelic variants on each of the numerous participating mol ecules. Therefore, the coding genes in their germline configuration play a primary role in determining who is at risk for developing such disorders, how the disease progresses, and how someone responds to therapy. Although genetic components in these diseases are clearly present, the lack of obvious and homogeneous modes of transmission has slowed progress by prevent ing the full exploitation of classical genetic epidemiologic techniques. Furthermore, autoimmune diseases are characterized by modest disease risk heritability and m- tifaceted interactions with environmental influences. Yet, several recent discoveries have dramatically changed our ability to examine genetic variation as it relates to human disease. In addition to the development of large-scale laboratory methods and tools to efficiently recognize and catalog DNA diversity, over the past few years there has been real progress in the application of new analytical and data-manage ment approaches.
