On 15 November 1997, the first international symposium "Neural and Chemical Control of Breathing: Pharmacological and Clinical Aspects" was held at Leiden University Medical Center on the occasion of the retirement of one of the members of the Control of Breathing Research Group of the Departments of Physiology and Anesthesiology, Dr. Aad Berkenbosch. Among others, Dr. Berkenbosch, played an important role in this research group, which made a large and significant contribution to scientific research on the regulation of breathing. This book presents the proceedings of that meeting together with papers of several authors who have strong bonds with the Leiden Departments of Physiology and Anesthesiology. All studies represent state of the art work on the subject of respiratory control and cardiovascular medicine, with emphasis on the physiological, pharmacological and anesthesiological aspects of both fields. The book is divided in several sections: Oxygen Physiology. Prof. John Severinghaus presents among other things his ideas on oxygen sensing and high altitude related diseases. Prof. Honda gives results from studies in a unique set of subjects without carotid bodies. The findings of Honda's group support the idea that an intact hypoxic drive from the carotid bodies is necessary for the generation of central hypoxic depression. Dr. Hans van Beek discusses the effects of hypoxia and hypercapnia on cardiac contractility in animal models. Oxygen levels are not stable in aquatic environments and exclusively water-breathing ?sh must still cope with the ever-changing levels of O 2 and with large temperature changes. This is re?ected in their sophisticated count- current systems, with high O extraction and internal and external O receptors. 2 2 The conquest for the terrestrial environment took place in the late Devonian period (355–359 million years ago), and recent discoveries portray the gradual transitional evolution of land vertebrates. The oxygen-rich and relatively stable atmospheric conditionsimpliedthatoxygen-sensingmechanismswererelativelysimpleandl- gain compared with acid–base regulation. Recently, physiology has expanded into related ?elds such as biochemistry, molecular biology, morphology and anatomy. In the light of the work in these ?elds, the introduction of DNA-based cladograms, which can be used to evaluate the likelihood of land vertebrates and lung?sh as a sister group, could explain why their cardio-respiratory control systems are similar. The diffusing capacity of a duck lung is 40 times higher than that of a toad or lung?sh. Certainly, some animals have evolved to rich high-performance levels.

The carotid body (CB) is in charge of adjusting ventilatory and cardiovascular function during changes in arterial blood gases. Regardless this essential function, the CB has been implicated in the sensing of other physiological signals such as changes in blood flow and glucose levels. More important, malfunction of the CB chemoreceptors has been associated with the progression and deterioration of several disease states such as hypertension, heart failure, renal failure, insulin resistance, diabetes and sleep apnea. Although the mechanisms involved in the alterations of the CB function in pathophysiology are currently under intense research, the development of therapeutic approaches to restore normal CB chemoreflex function remains unsolved. Recent studies showing the effect of CB denervation in pathophysiology have unveiled a key role of these arterial chemoreceptors in the development of autonomic imbalance and respiratory disturbances, and suggest that targeting the CB could represent a novel strategy to improve disease outcome. Unfortunately, classical pharmacotherapy intended to normalize CB function may be hard to establish since several cellular pathways are involved in the CB dysfunction. Augmented levels of angiotensin II, endothelin-1, cytokines and free radicals along with decreases in nitric oxide had all been related to the CB dysfunction. Moreover, changes in expression of angiotensin receptors, nitric oxide synthases and cytokines that take place within the CB tissue in pathological states also contribute to the enhanced CB chemoreflex drive. It has been shown in heart failure, hypertension and obstructive sleep apnea that the CB becomes tonically hyper-reactive. During the progression of the disease this CB chemosensory facilitation process induces central nervous system plasticity. The altered autonomic-respiratory control leads to increased cardiorespiratory distress and the deterioration of the condition. The focus of this e-book will be to cover the role of the CB in pathophysiology and to provide new evidence of the pathways involved in the maladaptive potentiation of the CB chemoreflex function. Since 1959, the International Society of Arterial Chemoreception (ISAC) has organized in a variety of countries fifteen scientific meetings devoted to the mechanisms of peripheral arterial chemoreception and chemoreceptor reflexes. After the meeting held in Philadelphia with Sukhamay Lahiri as president, ISAC membership elected Lyon (CNRS, University Claude Bernard, France) as the site of the xv" ISAC Symposium. The Symposium was effectively held in Lyon from the 18th to the 22nd of November 2002 and Jean-Marc Pequignot was its president. The organizers were Jean-Marc Pequignot and Yvette Dalmaz Lyon (CNRS, University Claude Bernard, France) and the Scientific Committee was formed by John Carroll (University of Arkansas for Medical Sciences, USA), Constancio Gonzalez (University of Valladolid, Spain), Prem Kumar (University of Birmingham, U. K. ), Sukhamay Lahiri (University of Pennsylvania, Philadelphia, USA), Colin Nurse (McMaster University, Hamilton, Ontario, Canada), and Nanduri Prabhakar (Case Western University, Cleveland, Ohio, USA). The Symposium in Lyon intended to follow the path opened in Philadelphia gathering people working at the interface of cellular and molecular biology with researchers in the more classical topics of chemoreception pathways and reflexes. The aim was to join experts with different perspectives. Along these lines, some participants are engaged in the exploration of the intimate mechanisms of oxygen sensing and cellular responses, with their work centered in a great number of preparations covering a broad spectrum from bacteria, to chemoreceptor cells or to central nervous systems neurons.
