This book aims to provide a single reference source on levodopa-induced dyskinesias (LID) from ‘bench to bedside’. Initial chapters review the clinical features and phenomenology of LID with video examples; epidemiology and genetic risk factors for LID are covered as a background to understanding risk factors for developing LID. The following chapters cover the latest preclinical studies aiming to understand the pathophysiology of LID at the cellular, neurochemical, neurophysiological and circuitry level with detailed discussion of mechanisms and future directions to take the field forward; clinical studies from phase II to phase IV; on going RCTs in LID and evidence-based medicine reviews of treatment options. Levodopa-Induced Dyskinesia in Parkinson’s Disease is aimed at an international audience of movement disorder neurologists; neuroscientists; trainees and graduate and post-graduate students.

For many years, the need to develop valid tools to evaluate signs and symptoms of Parkinson Disease (PD) has been present. However the understanding of all intricacies of rating scales development was not widely available and the first attempts were relatively crude. In 2002, the Movement Disorders Society created a task force to systemize the measurement of Parkinson's Disease. Since then, the Task Force has produced and published several critiques to the available rating scales addressing both motor and non-motor domains of Parkinson Disease. Additionally the task force initiated a project to develop a new version of the UPDRS, the MDS-UPDRS. But none of this was made available in one convenient source. Until now. Rating Scales in Parkinson's Disease: Clinical Practice and Research is written for researchers from the medical and social sciences, and for health professionals wishing to evaluate the progress of their patients suffering from Parkinson's Disease. The book is both exhaustive in the description of the scales and informative on the advantages and limitations of each scale. As such, the text clearly guides readers on how to choose and use the instruments available. Extensive cross-referenced tables and charts closely integrate the parts of the book to facilitate readers in moving from one symptom domain to another.

Levodopa-induced motor complications are extremely prevalent (affecting over half of all PD patients after 4-6 years of levodopa therapy), disabling, and one of the more challenging to treat of all PD symptoms. Motor fluctuations result in the re-appearance or a delay in relief of motor symptoms, following levodopa administration. Motor fluctuations occur more frequently as PD progresses and "on" periods (when motor symptoms are well-controlled) typically get shorter and "off" periods (when motor symptoms re-emerge) get longer. Levodopa-related dyskinesia can also be disturbing and disabling. There are two major types of dyskinesia - "peak dose" and "diphasic" - which are manifested by variable sequences of parkinsonism, symptom improvement, and dyskinesia. Treatment strategies for dyskinesia include adjustments in the timing and dose of levodopa, the use of amantadine (an agent with dopaminergic, NMDA receptor antagonist, norandrenergic and possibly anticholinergic activity) and deep brain stimulation surgery. Part of the Oxford American Pocket Notes series, this volume is designed for general neurologists who are often the first physicians to receive complaints of motor fluctuations and dyskinesia in PD patients. This pocket guide serves as an invaluable resource in aiding in the recognition and management of motor fluctuations and dyskinesia, and ultimately, improving the overall quality of life of persons with PD.
