Mammaglobin (MMG) is a breast-specific glycoprotein that is over-expressed in 80% of all primary and metastatic breast cancer. Despite its uniqueness as a breast-specific cancer marker, the utility of MMG in breast cancer imaging has not been explored. Consequently, the goal of this project is to evaluate the potential of imaging the expression of MMG in established human breast cancer models. TO accomplish this goal, we labeled polyclonal and monoclonal anti-MMG antibodies with a near infrared fluorescent probe for optical imaging and 64Cu-DOTA for positron emission tomography (mPET). Preliminary results indicate that the mPET imaging with 64Cu-DOTA-anti MMG monoclonal antibodies showed predominant liver uptake in mice. In contrast, the fluorescent-labeled derivative of polyclonal MMG antibody primarily accumulated in the tumor, kidneys and liver. We have also developed a tumor model with increased expression of MMG. Our preliminary results appear to support the hypothesis that targeting the expression of MMG will enhance the localization of breast cancer tumors.

Review use of exploiting differences in tissue autofluorescence to diagnose and treat cancer. Include agents such as 5-aminoleculinic acid. Review mechanisms that require proteolytic processing within the tumor to become active fluorophores. Review use of cancer selective proteins to localize probes to cancer cells: include toxins, antibodies, and minibodies. Introduction of plasmids, viruses or other genetic material may be used to express fluorescent agents in vivo. This chapter will review multiple vectors and delivery mechanisms of optical imaging cassettes.Preclinical investigations into the use of optical contrast agents for the detection of primary tumors in conventional and orthotopic models will be discussed. Preclinical investigations into the use of optical contrast agents for the detection of metastatic tumors in mouse models will be discussed. Use of targeted and non-specific optical contrast agents have been used for the detection of sentinel lymph node detection. These applications and how they differ from other applications will be discussed. Because of the unique difficulty of identifying tumor from normal tissue in brain tissue, a separate chapter would be needed. More clinical data is available for this cancer type than any other. Discussion of potential clinical applications for optical imaging and an assessment of the potential market.

The primary objective of this research program is to investigate concurrent near infrared (NTR) optical and magnetic resonance (MR) imaging for breast cancer diagnosis. The NIR diffuse optical imaging offers novel criteria for cancer differentiation with the ability to measure (in vivo) oxygenation and vascularization state, the uptake and release of contrast agents and chromophore concentrations with high sensitivity. However, NIR diffuse optical tomography is inherently a low spatial resolution imaging modality due to diffuse nature of light photons. Alternatively, MRI provides high spatial resolution with excellent tissue discrimination, but has limited ability to monitor hemoglobin dynamics and other contrast mechanisms that optical imaging provides. Therefore, concurrent MRI-NIR optical imaging brings together the most advantageous aspects of the two imaging modalities for breast cancer diagnosis.
