Myocardial ischemic syndromes pose a major medical problem and a significant economic health care concern. Reperfusion, although used in the clinical arena as essential to the survival of acutely ischemic heart muscle carries with it the risk of "reperfusion injury". Therefore the salvage of additional myocardium is highly desirable. Over a decade ago, it was shown that whole body heat shock activated a powerful endogenous protective mechanism that significantly improved myocardial salvage following prolonged ischemia and reperfusion injury in the heart. A characteristic feature of this heat shock response was the expression of a family of proteins known as heat shock proteins. Many of these proteins function as molecular chaperones, helping to refold proteins denatured as a consequence of lethal injuries and facilitating the synthesis of new proteins to replace those irreparably damaged. Altered expression of these proteins has been extensively documented in association with a diverse array of diseases such as ischemia and perfusion injury, cardiac hypertrophy, fever, inflammation, metabolic diseases, infection, cell and tissue trauma, aging, and cancer. Interestingly, some studies have now shown a direct correlation of the heat shock proteins produced and the degree of myocardial protection. This book is a thorough description of the current state of knowledge of the mechanisms of heat shock proteins induced cardiac protection at the cellular and molecular level, the controversies in this growing field and the potential of treating ischemic heart disease with overexpression of heat shock proteins in patients. The book will be especially useful for investigators interested in studying tissue injury and its protection with the overexpression of heat shock proteins.

Heat shock proteins (HSP) were originally described in heat-shocked drosophila by Ritossa in the early 1960's. In the last 5 years it has become apparent that these heat shock proteins have important functions both in the normal cell and in the stressed cell. These proteins constitute an endogenous protective system; mutations in the heat shock proteins can be lethal, and there are no known organisms without heat shock proteins. The first observations on heat shock proteins and the heart were made in the 1980's and interest in these proteins increased over the decade. In the last few years there has been an exponential growth in number of papers published on heat shock proteins and the heart. Heat shock proteins have been implicated in a number of cardiovascular research areas including ischemia, hypertrophy, aging, and atherosclerosis, and this list is growmg. This research effort is directed at subobjective 2 as defined in the recent shock trauma mediators review memo from the U.S. Army Institute of Surgical Research. Specifically, we aimed at the prevention of secondary damage after hemorrhage by temporizing fundamental physiological or biochemical processes leading to cell death and organ failure. For this purpose, increased expression of the inducible heat shock 70 (hsp70I) will be employed. Hsp7OI works as a chaperone attaching to short hydrophobic peptide sequences as they are exposed in ischemic cells in which proteins undergo denaturation. By the association of hsp70 with such protein sequences further protein aggregation and cell damage is prevented. This leads to a faster recovery of cell function after the ischemic or hemorrhage episodes. Our results which are further summarized in the body indicate that overexpression of hsp70 in the heart of transgenic mice leads to marked protection against cardiac ischemic injury (1). In preliminary studies in collaboration with Dr. P. Lyden, UCSD and Dr. B. Nishimura, UCLA, we have found that the hsp70 transgene is also expressed in CNS-neurons and exerts a protective effect against ischemic damage in the brain as judged by behavioral scores in these transgenic mice.
