Circadian rhythms are the daily fluctuations in physiological processes that govern cell cycles and timing of behaviors. Cancer hallmarks are the cellular properties that drive the perennial growth, survival and spread of cancerous cells. The disruption of circadian rhythms can contribute to the development of some cancers by affecting the expression of genes that regulate cell growth and division, as well as their interaction with the host microenvironment and anti-cancer treatment. Some cancer cells have been found to have altered circadian rhythms compared to normal cells, which may contribute to their uncontrolled growth. Patients exhibiting disrupted circadian rhythms tend to suffer accelerated tumor growth and metastasis, tumors that are more resistant to treatment, and poorer health-related quality of life and overall survival. Circadian rhythms also temporally regulate cellular immunity, which has important implications for the burgeoning field of immune-based therapeutics. Further research is needed to better understand the relationship between circadian rhythms and cancer hallmarks, and how this knowledge can be used to improve immunotherapy outcomes, cancer patients’ well-being and survival. This timely research topic is focused within a field that produced two Nobel prize awards in Physiology or Medicine, in 2017 for circadian rhythm research, and in 2018 for the discovery of cancer immunotherapy. The aim of this special issue is to highlight clinical, translational and epidemiological science - that includes objective assessments of both circadian disruption and cancer hallmarks - in order to help elucidate the impact of circadian rhythms on processes associated with cancer and its treatments, immunotherapy in particular.

Over the years, there has been an increasing demand for an effective and cost-efficient drug discovery process. The study of old and previously-approved drugs for newer indications or their re-evaluation based on newly-identified different mechanisms of action has become a major focus of drug discovery, particularly when it comes to treating chronic or complex multigenetic diseases such as cancer, autoimmune, neurodegenerative disorders, and psychiatric conditions. This approach is actually particularly appealing since it saves time and resources and reduces the risk of failure. The history of pharmacology is replete with examples of drugs that have undergone significant shifts in their clinical or preventive use over time, such as acetylsalicylic acid, thalidomide, duloxetine, and dapagliflozin. For instance, acetylsalicylic acid has been used for over a century as an anti-inflammatory and analgesic, but it is currently used as an antiplatelet agent. Thalidomide was previously removed from the market because of its teratogenic effects which were associated with the inhibition of the synthesis of tumor necrosis factor-alpha and other cytokines, but it has since emerged as an immunomodulator for the treatment of various diseases, including erythema nodosum leprosum. Intriguingly, repurposed drugs have also been shown to control synaptic potentiation associated with neurological and psychiatric disorders, which have opened unimaginable scenarios a few years ago.

Papillary thyroid cancer (PTC) is by far the most common thyroid malignancy, reaching a prevalence of about 80%. Furthermore, it represents the unique responsible for the increasing thyroid cancer incidence. Upon thyroid ablation (thyroidectomy with or without iodine-131 administration), PTC has an excellent prognosis with nearly 100% 5-year disease-specific survival. However, about 25-30% of patients experience disease morbidity, as related to persistent structural disease/recurrence upon initial standard treatment, and 10% die over time, as related to cancer. Furthermore, the latest data from the US Surveillance, Epidemiology, and End Results found that disease-related mortality is slightly increasing over time.

There is a huge amount of literature on statistical models for the prediction of survival after diagnosis of a wide range of diseases like cancer, cardiovascular disease, and chronic kidney disease. Current practice is to use prediction models based on the Cox proportional hazards model and to present those as static models for remaining lifetime after diagnosis or treatment. In contrast, Dynamic Prediction in Clinical Survival Analysis focuses on dynamic models for the remaining lifetime at later points in time, for instance using landmark models. Designed to be useful to applied statisticians and clinical epidemiologists, each chapter in the book has a practical focus on the issues of working with real life data. Chapters conclude with additional material either on the interpretation of the models, alternative models, or theoretical background.
