During the early 19th century, it was discovered that adding yellow (now called white) phosphorous to matchstick heads made it easier to ignite matches. The phosphorous vapors were breathed in by workers and combined with other chemicals in the body to produce a potent nitrogen-containing bisphosphonate. Today's oral nitrogen-containing bisphosphonates and intravenous nitrogen-containing bisphosphonates circulate around the body the same way as the phosphorous vapors, and are absorbed into bone and ingested by osteoclasts. When this unique binding process of bisphosphonates to bone occurs, osteoclasts are poisoned, and this reduces or eliminates bone turnover. Alveolar bone in the mandible and maxilla turns over more rapidly than in long bones, so the jaws are a better target for bisphosphonate toxicity. It wasn't until 2003 that today's intravenous and oral nitrogen-containing bisphosphonate medications were implicated as major risk factors in the development of exposed necrotic bone of the jaws. Most of the researchers who reported cases of bisphosphonate-induced osteonecrosis of the jaw found that these patients were treated with zoledronate, pamidronate, or a combination of these drugs, which are commonly used for treating breast cancer or myeloma. In about 5% of cases, subjects with BIONJ were being treated for osteoporosis. Precipitating events that contribute to BIONJ are tooth extractions (about 50% of cases), mandibular exostoses, periodontal disease, and local trauma from ill-fitting dentures. It is not known if the placement of dental implants is a precipitating factor. The book aims to meet the need of medical practitioners working in all fields that use bisphosphonates, and to present the conservative and surgical treatment methods currently in use. There will also be detailed information on the literature relating to dental implants in patients treated with bisphosphonates.

The results of randomized trials evaluating the use of early or adjuvant systemic treatment for patients with resectable breast cancer provide an eloquent rebuttal to those who would argue that we have made no progress in the treatment of cancer. Many of the tumors that we have been most successful in curing with chemotherapy and other newer forms of treatment are relatively uncommon. In contrast, breast cancer continues to be the single most common malignancy among women in the western world, is increasingly a cause of death throughout Asia and Third-World countries, and remains one of the most substantial causes of cancer mortality world wide. The use of mammography as a means of early detection has been shown to reduce breast cancer mortality by 25-35% among those popu lations in which it is utilized. The use of adjuvant systemic treatment in appropriate patients provides a similar (and additional) reduction in breast cancer mortality. Few subjects have been so systematically studied in the history of medicine, and it seems fair to conclude that the value to adjuvant systemic therapy in prolonging the lives of women with breast cancer is more firmly supported by empirical evidence than even the more conventional or primary treatments using various combinations ofsurgery and radiotherapy.

The potential capacity of the biphosphonates to intervene in osseous remodeling was recognized more than 30 years ago. The biphosphonates inhibit osteoclastic activity. In spite of advances in early detection, surgey, chemo-and radiotherapy, metastatic bone disease is still a major and in many cases intractable clinical problem. The biphosphonates are now the most effective agents in the management of bone metastases - as shown by trials in patients with breast and prostate cancer, and in multiple myeloma. Bone pain, new ostelytic events and pathologic fractures are decreased and complications associated with these events are avoided. Existing osteolytic lesions may even be reversed. New fields of indications of biphosphonates are shown to be in the adjuvant therapy of breast cancer and in stage 1 multiple myeloma to prevent skeletal destruction and also to inhibit tumor growth. Over the past few years the biphosphonates have proved their value also in many other conditions such as Paget's disease of bone, osseous disorders with an osteoclastic component leading to osteopenia/osteoporosis, in congenial, endocrine and metabolic disorders. Perhaps, for the future and for the next millennium the most widespread and extensive use to these anti-osteoclastic component leading to osteopenia/osteoporosis, in congenial, endocrine and metabolic disorders.
